Appointments and Registration

Appointments may be scheduled at any of our six locations Monday through Friday from 8:00am-5:00pm. If you are a new patient, we ask that you arrive at least 15 minutes early to fill out necessary paperwork. Paperwork may also be mailed to you in advance to facilitate the process. We also ask that you provide us with as much medical information as possible, such as recent lab or x-ray reports. Please consult with your primary care physician to release your medical records and forward them to our office in advance.

Hospital Affiliations

Our physicians are board certified in Cardiovascular Diseases, Nuclear and Interventional Cardiology and are proud to be affiliated with the North Shore/LIJ Health System, Winthrop University Hospital, and New York Hospital Queens.

Payment Options

Long Island Heart Associates accepts most major insurance plans. For your convenience, we accept cash, checks or credit cards for co-payments. Please be sure to bring your insurance card with you at the time of your appointment. If you are covered by an HMO, you must bring your referral with you.
